Evaluation and Rehabilitation to Low Back Disorders

 

Abstract:

Physical loads concentrate on the lower lumbar vertebrae during repetitive sports activities, and initially feel uncomfortable, and microinjury occurs at specific tissues due to repetitive loading, resulting in Low Back Pain during exercise. Although symptoms improve by sufficient rest and proper conditioning at this stage, inflammation occurs when the load is continued, and LBP occurs even after the exercise. Continued sports activities further cause such as bone resorption or proliferation, eventually leading to deformation. 

 The application of extension load to the lumbar region causes facet joint disorder, spondylolysis, sacroiliac joint disorder, and flexion compression load causes intervertebral pain. In addition, the load to myofascial tissue causes myofascial pain syndrome or trunk muscle strain, repetitive traction load causes an enthesopathy at the attachment of the muscle to the bone.

 Block injection or medication are effective to acute and sub-acute LBP, manual therapy for the joint disorder, massage or acupuncture are effective to muscle hypertonus. However, in order to decrease the load to the specific tissues, it is necessary to prescribe proper Athletic Rehabilitation for the purpose of expanding the ROM of the adjacent joints of the lumbar spine, improving trunk stabilization function and improving the motor control function.

 

 

Background:

Koji Kaneoka, Orthopedic doctor, Sports doctor specialized in the spine, who graduated from Tsukuba University in 1988. After serving an instructor at Orthopedic Surgery at University of Tsukuba, he became an associate professor in 2007 and currently works as a professor at Waseda University from 2012. He served as the official team doctor for Japan National Swimming at Sydney, Athene, and Beijing.  He also was in charge of Sports Medicine doctor unit of JOC at London Olympics.
